Concussion Insurance is an active supplemental insurance policy for the registered players in the Phoenix Rising FC Youth Soccer Competitive Program (10/15/2023 – 10/15/2024). There is no need to enroll! Phoenix Rising FC Youth Soccer Competitive Program players automatically have access to this program, courtesy of Phoenix Rising FC Youth Soccer.
Insurance Coverage
This policy goes into effect once a youth athlete has been diagnosed with a concussion. Once a concussed player’s personal insurance coverage is exceeded, certain additional medical expenses related to the concussion — including the post-injury test — may be billed to an excess insurance policy that’s included in the program. In the absence of any other coverage, this plan will provide primary coverage benefits up to the policy’s maximum limitations and exclusions. This coverage may help lessen the potential financial burden on families. This program’s insurance policy covers the treatment of concussions and offers a maximum medical benefit of $25,000 per injury, with no deductible and no copays. Please confirm insurance information with all providers you work with to manage coverage and expectations when an injury situation occurs. Always ask your provider(s) what procedures / therapies / services / facilities / transportation may or may not be covered by this insurance as this insurance is a supplemental (limited pay) policy and does not cover all procedures / therapies / services / facilities / transportation. Emergency room visits and transportation may or may not be covered with this policy.
Benefits of the Excess Policy
- Access to trained medical providers for injury evaluation
If an athlete appears to have sustained a concussion during Phoenix Rising FC Youth Soccer approved events (League Games, Tournaments, Practice, Approved Team Activities, and Scrimmages), he or she is immediately pulled from play. The athlete is referred for clinical assessment and specialty care by a medical professional, Medical Doctor (MD) or Doctor of Osteopathy (DO), who is trained in the evaluation and management of concussions. The athlete cannot return to soccer activities until they have been cleared by a medical professional, Medical Doctor (MD) or Doctor of Osteopathy (DO) to return to full participation without restrictions.
- Post-injury testing
The athlete then undergoes a post-injury test and evaluation and the results are compared to any available baseline test or age-appropriate normative data to help the medical professional, Medical Doctor (MD) or Doctor of Osteopathy (DO), determine the scope and magnitude of the concussion.
- Follow-up and safe return to play
The medical professional, Medical Doctor (MD) or Doctor of Osteopathy (DO), provides the athlete with an individual follow-up plan that may include rest, gradual re-exertion, and re-evaluation. The athlete continues with follow-up care until cleared by a qualified medical professional, Medical Doctor (MD) or Doctor of Osteopathy (DO), for safe return to play. When medically cleared, the athlete will follow Phoenix Rising FC Youth Soccer’s Return to Play Progression.
Making a Claim
If it has been determined that your Player has sustained a possible concussion during Phoenix Rising FC Youth Soccer Competitive program approved events (League Games, Tournaments, Practice, Approved Team Activities and Scrimmages), you MUST immediately contact Brad Clement, Phoenix Rising FC Youth Soccer VP of Competitive, at bradc@prfcyouthsoccer.com to begin the Documentation / Insurance / Diagnosis / Treatment / Clearance / Return to Play process.
- A Phoenix Rising FC Youth Soccer Concussion Injury Packet will be provided to the family as well as a Concussion Insurance Claim Form.
- Completed Claim Forms, filled-out and submitted by the Player’s Parent / Guardian with all questions answered and all boxes in all sections completed, must be received by the insurance company (The Hartford) within ninety (90) calendar days of the incident. The Club Administrator must sign and date this Claim Form.
- Medical expenses incurred while under the care of the medical professional, Medical Doctor (MD) or Doctor of Osteopathy (DO), are submitted to your primary health insurance provider.
- Itemized bills will be submitted to the insurance company directly from your physician’s office. Your medical provider, Medical Doctor (MD) or Doctor of Osteopathy (DO), may contact the program’s insurance company, The Hartford at (888) 998-2240.
